Post curl PHP z załącznikiem; niestandardowy nagłówek typu zawartości

Muszę wysłać plik na serwer wraz z kilkoma postami. Dokumentacja otrzymana od tego administratora serwera pokazuje przykład, jak ma wyglądać prośba o wpis (* UWAGA niestandardowy typ zawartości dla POST „wieloczęściowy / x-api-zdalny-integracja”):

Z tego, co odbieram z serwera, wygląda na to, że pola pocztowe są prawidłowo odbierane. Jednak jest to plik, który nie jest odbierany. Nie wiem, co zrobiłem źle. Wszelkie dane wejściowe są bardzo mile widziane.

Wiem, że nazwa użytkownika i dane pw są poprawnie odczytywane przez serwer, co przetestowałem z błędnymi informacjami i zamiast tego otrzymałem błąd poświadczenia.
