Estou tentando adicionar valores a uma tabela no phpmyadmin e recebo o erro: Coluna desconhecida '...' na 'lista de campos'.

Aqui está meu código:

Então, quando eu insiro fds como nome no formulário da página anterior eu recebo: Coluna desconhecida 'fds' na 'lista de campos'. Isso nunca aconteceu comigo antes e eu não tenho idéia do que está acontecendo.
